Allow to create a loopback cdev from a file.

Changes since v1:
- Renamed cdev_create_from_file to cdev_create_loop
- Added cdev_remove_loop, which replaces cdev_close and also removes the cdev
and frees its memories.
- Actually increment loopno with each call to cdev_create_loop
